Continuation programs must download the Measurable Objective Status Report (Form JCPC /MOR 001), complete and attach this report to the JCPC Program Agreement for FY 2010-2011 upon submission. Information about this form is included in the instructions section of the program agreement. Instructions also are provided for programs applying for these funds for the first time.
Not for profit organizations must download the Conflict of Interest Form (DJJDP 13 001), complete and attach this report to the JCPC Program Agreement for FY 2010-2011 upon submission.
Every Grantee must download the State Grant Certification-No Overdue Tax Debts form, complete and attach this form to the JCPC Program Agreement for FY 2010-2011 upon submission.
